This position will be in our In-Home Service department of Breakthrough in Home Service IHS. We are looking for skilled professionals to provide services in the comfort of the client's home (HHA, CNA, LPN & RN).

Responsibility:

·      Patient assessments

·      Health monitoring

·      Training of family members to perform some procedures

·      Homemaker chores

·      Advance personal care

·      Respite

Requirements:

·      Age 18 and over

·      Current, unencumbered state certification to practice as a Certified Nursing Assistant (Required for CNA’s Only)

·      Must have a valid Driver’s License, State ID or Passport

·      Must have Social Security Card or Birth Certificate

·      Must be registered on the Family Care Safety Registry (FCSR) -
-      Education/License: CNA, CPR Certified

-      Experience: Minimum of 6 months experience required
